Esto es ilegal no sólo porque los servicios médicos están exentos de IVA (artículo 15 de la Ley del Impuesto al Valor Agregado), sino porque, en general, no está permitido el cobro de un pago extra por requerir factura (todos los precios de cualquier producto o servicio que sea causante de IVA deben incluir este impuesto desde el inicio).  They do not issue an invoice, they deceptively give you a price for studies and when you request an invoice they say that it does not include an invoice and it will be prices plus VAT.
This is illegal not only because medical services are exempt from VAT (article 15 of the Value Added Tax Law), but because, in general, it is not allowed to collect an extra payment for requiring an invoice (all prices of any product or service that causes VAT must include this tax from the beginning).
